Local Care Direct has been leading the way in the development of innovative services by supporting people with long term conditions at home, empowering them to manage their own health and avoiding unnecessary hospital attendances and admissions.
Over the past two years Local Care Direct has delivered a Telehealth service.
By linking the latest remote-monitoring technology directly to our specialist clinical support team we have helped to care for people with chronic health needs in their own home – 24 hours a day, seven days a week.
Our cutting edge healthcare service was piloted in Leeds in partnership with Alere Connected Health. It provided a menu of services for patients with Chronic Obstructive Pulmonary Disease (COPD) and Chronic Heart Failure (CHF) which monitored vital signs including blood pressure, heart rate, weight and blood oxygen saturations.
Patients told us they really liked the service. Clear evidence was gathered of the improved health outcomes the scheme achieved and this generated interest among health commissioners and business partners across the country.
Our Telehealth service led us to become the winner of the 2013 Social Enterprise UK Award for Innovation, and we have recently been nominated as a finalist in the Medipex NHS Innovation Awards for Software and Telehealth to be held on 9th October 2014.
The project finished at the end of July 2014.
