Local Care Direct operates three Practices across West Yorkshire serving diverse communities with challenging health needs:
Manningham Medical Practice
Located in Bradford, this Practice requires a bespoke approach to patient care which reflects both the health needs and the rich multicultural diversity of the local community. Local Care Direct engages with community leaders via a regular Patient Forum to ensure the requirements of the patients are addressed effectively. Manningham has been chosen as one of the pioneering multidisciplinary integrated care sites in Bradford.
Hillside Bridge Health Centre
This Health Centre is also in Bradford and was the country’s first Equitable Access Centre providing both a GP practice for registered patients and a walk-in service. Here, Local Care Direct successfully implemented a new model of telephone triage to improve patient access to their GP ensuring that the patient sees the right person, in the right place, at the right time.
King Street Health Centre
This Health Centre is in Wakefield and offers general practice and a walk-in service that is well established within the local health economy and popular with residents of Wakefield district. With a GP and prescribing Advanced Nurse Practitioners we can see a wide range of semi-acute presenting conditions and minor illnesses. This acts as the step in between a patient’s own registered GP and the Accident & Emergency department.
